How to apply

  1. Fill out the Jack Bennett Fund application form.
  2. Gather a photo of the grave marker showing the veteran's name clearly and proof of the next of kin's income.
  3. Mail everything in one envelope to the WV Department of Veterans Assistance, Office of the Cabinet Secretary, in Charleston.

Deadline: No fixed deadline. Families can apply any time after the marker is installed, and approval depends on meeting the VA's income means test. (Rolling, apply any time)
