- Complete the cemetery's burial application form, available as a downloadable PDF on the cemetery website. Pre-registration is optional; you do not need an application on file in advance.
- Provide a copy of the discharge document (DD Form 214 for active duty or NGB Form 22 for Guard or Reserve) so the cemetery can verify eligibility before interment.
- Contact the cemetery office at 701-667-1418, located at 4606 Highway 1806, Mandan, ND 58554, or work with your funeral home to coordinate the burial.
